Use this form to request a badge for New Staff, Volunteers, Vendors, Replacement Badges, or to change access rights.

Badges are a Digital Master Key  to Life.Church locations. Please notify Security, via this form, if a badge needs to be deactivated or is lost/stolen.

**ATTENTION Please read!!**

Volunteer badge guidelines:

  • If a volunteer serves during normal business hours or service times, or when staff is present, they do not need a badge.
  • We’re asking for a max of 15-20 volunteer badges for your campus.
  • Requests for volunteer badges will now include the requestor to provide operational reasoning for the badge, as well as an approval from the CP. These steps will be added to the current badge request format and will prompt the requestor to provide this information as they navigate through the request.
  • The volunteer badge design has changed to clearly identify volunteers. All new and replacement badges will be using the new template.

Non-volunteer related badge guidelines:

  • Security officer badges will continue as normal and do not count towards the 15-20 limit. Please continue to fill out the badge request form for new officers
  • Vendors (cleaning crew, maintenance, etc.) will not count against the 15-20 badge limit
  • We will no longer be issuing spouse badges

According to the updated badge policy, all volunteers must complete a Leader Acknowledgement Form prior to obtaining a badge. If your volunteer has not completed this form yet, please do so before completing this request.

Reminder: If this is for a Volunteer Safety Team Member, please go back and choose "Volunteer" in the dropdown. Security badges are only for Contract Security Officers.

Does volunteer have a current/approved background check?*

Per Volunteer Policy, all volunteers MUST have an approved back ground check BEFORE serving.
