DUE DATES
STANDARD REQUESTS: Please note standard turn-around time on request tickets is 2 weeks. If a unique or specific due date is needed it will be honored within reason and to the best of our ability.
URGENT REQUESTS: If needed within the next 48 hours please call Keast Neal at 405-706-2772, in addition to completing this form.
DEFINITIONS
ITEM REMOVAL: Removal and/or disposal of LC furniture or equipment, with the original cost of the item exceeding $50, that is no longer being used at your location. Example: A laptop that needs to be returned to Central IT or furniture that is no longer needed. Note: Please upload a picture of all items to be removed to help us determine where to take it or dispose of it.
MOVE IT: Any LC owned item that needs to be moved from one LC location to another LC location. Example: Furniture request being fulfilled to deliver to your office.
SHIP IT: Any LC item that needs to be shipped. Example: A package that needs to arrive at a regional campus for an upcoming event. Note: When shipping internationally please provide the complete address, contact name, phone # and email address, along with any special instructions to help the package arrive in a timely manner.
